If you live with a chronic condition such as diabetes, asthma, arthritis or heart disease, your GP can help you manage it with a care plan.

What changed?

From 1 July 2025, GP Management Plans and Team Care Arrangements were replaced by a single GP Chronic Condition Management Plan. You're eligible if you have a condition that has lasted, or is likely to last, six months or more.

What it means for you

  • Your plan sets out your goals and the care you need, and your GP reviews it regularly.
  • Our practice nurses can help prepare and review your plan with your GP.
  • Your GP can refer you to allied health providers. Medicare rebates are available for up to five allied health services per calendar year.
  • Allied health referrals are now made by referral letter. Medicare provides a rebate for eligible services, but allied health providers may set their own fees, so there may be an out-of-pocket cost.

Already have an older plan?

Referrals written before 1 July 2025 stay valid until the services on them are used. When your plan is next reviewed, your GP will move you to the new plan.

MyMedicare patients

If you're registered with MyMedicare, your plan and its reviews need to be done through the practice you're registered with.

Allied health under one roof

We have a physiotherapist, dietitian, podiatrist and diabetes educator on site, which makes team care easier.
